ENROLLMENT FORM/REGISTER FOR A COURSE

- Fill out the form - and fax or email it to the DIVESAFE office - include the non-refundable registration fee. ( A non-refundable registration fee of $150.00 for CAN or $350.00 for International students is required with the enrollment form)

YOU NOW HAVE A SEAT RESERVED IN YOUR COURSE OF CHOICE
2. Meet Pre-requisites:
Send in copies of:
- Divers medical
- Recreational certification and 20 logged dives
3. Co-requisites: (Oxygen Therapy/1st Aid Course/WHIMS):
There are two options for acquiring these certificates:
- Achieve these certifcates on your own through local companies such as St. Johns First Aid, local College etc.
- These certifcates will be available to complete during your Commercial diver course and can be added to the price of the course. Let the DIVESAFE office know you will be attending the 1st aid courses at the school.
- WHIMS: Workplace Hazardous Materials Information System
- First Aid Course
- Oxygen Therapy Certificate
4. Pay Fees:
Payment in full must be received two weeks prior to the commencement of the course. Course fees must be paid in the following manner:
- Cheque or money order
- Cash
- VISA/MasterCard
Note 1: If you withdraw after full payment has been received, refunds are subject to the DiveSafe International refund policy. Please refer to the student handbook for details. The registration fee is non refundable.
